Our online account forms are the easiest way to open an account. You can download our New Customer Forms booklets directly from this page. These booklets contain all the information and forms we will need to get your account started. You will need Adobe Acrobat Reader to download the forms. If you don’t already have Adobe Acrobat Reader, a free download is available on this page.

Once Acrobat is installed, you may download your account forms by simply clicking below on the type of account you wish to open. When the forms are displayed on your screen you may print and complete them. Most clients are able to complete the forms in as little as 15 minutes. Account forms may be faxed or mailed to us (Please note the additional information at the bottom of this page for details). Select the document(s) that apply to you. (If downloading documents, please allow between 25 - 45 minutes to complete the process)

 

Before completing the forms, simply refer to the PFG Account Matrix (page 3 of the Customer Agreement) to determine which forms you will be required to complete. Please note that all information is to be hand-written. Make sure to keep a copy for your records.

3. Fund your account.

*Checks must be drawn on an account that is in the name of the account holder;

*A check for an individual account must be in the name of that individual.

*A check for a corporate account must be in the name of that corporation.
